Acknowledgment of Risk

I am the parent or legal guardian of the above-named child. I understand that participation in flag football and related activities involves inherent risks, including but not limited to:

Running, jumping, and physical exertion

Falls, slips, and collisions with other participants

Contact with equipment or playing surfaces

Sprains, strains, fractures, concussions, and other injuries

Weather-related conditions, including heat and dehydration

I understand that while every reasonable effort will be made to provide a safe environment, injuries may occur.

Release of Liability

In consideration for my child's participation, I voluntarily assume all risks associated with participation. To the fullest extent permitted by law, I release and hold harmless the event organizers, sponsors, volunteers, coaches, officials, property owners, and their respective employees, agents, and representatives from any and all claims, demands, damages, liabilities, or causes of action arising out of or related to my child's participation, except to the extent caused by gross negligence or willful misconduct.

Medical Authorization

If I cannot be reached in the event of an emergency, I authorize the event staff to obtain emergency medical treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses incurred.

Code of Conduct

I understand that:

Participants are expected to demonstrate good sportsmanship and respect toward coaches, officials, volunteers, and other players.

Unsportsmanlike behavior, fighting, bullying, or unsafe conduct may result in removal from the event without refund.

Parents and spectators are also expected to conduct themselves respectfully.
